✚ HEALTH & MEDICINE 3 MONTHS HORIZON RESOLVES 1 JAN 2027 · in 3mo

The World Health Organization characterizes any plague outbreak as a pandemic before the end of 2026.

Plague is caused by Yersinia pestis. Resolves if the WHO explicitly applies the label 'pandemic' to a plague outbreak in official communications by year-end 2026.

Across 6 frontier models answering independently, the panel has all but ruled it out: the median is 2%.

They broadly agree, 1 points between Claude at 2% and Gemini at 1%.

  • ClaudeAnthropic

Plague remains geographically contained with modern antibiotics, and WHO reserves 'pandemic' terminology for rapidly spreading novel pathogens.

    • Current plague cases (DRC, Madagascar) are localized, treatable outbreaks, not spreading widely across continents.
    • WHO has never labeled plague a pandemic historically despite periodic outbreaks, reflecting both epidemiology and terminology conventions.
    • Only a sudden, unprecedented global surge in pneumonic plague transmission over the next ~3 months would change this, which is highly improbable.

  • GrokxAI

Turns on a novel transmissible Y. pestis strain causing multi-continent spread that WHO labels pandemic in ~85 days.

    • WHO last applied “pandemic” after sustained global COVID transmission; plague stays enzootic with rare contained clusters (e.g. Madagascar 2017).
    • No unusual plague activity or PHEIC exists as of 7 Oct 2026, so detection-to-declaration would have to finish before 31 Dec.
    • New-pandemic base rate in any 3-month window is <<1%; this case sits even lower without a current seed event.

WHO has never declared plague a pandemic; it would require an unprecedented global spread of Y. pestis in under 3 months.

    • Plague causes ~1,000-3,000 cases/year globally, mostly localized in Madagascar, DRC, Peru—nowhere near pandemic scale.
    • WHO's pandemic declaration bar is extremely high (H1N1 2009, SARS-CoV-2 2020); plague has not approached that threshold in modern history.
    • With only ~3 months remaining in 2026, even a sudden outbreak would need to achieve sustained multi-continent transmission, which Y. pestis has not done since the 14th…

Plague is easily treated with standard antibiotics, making a rapid global spread prompting a WHO pandemic designation in 2026 almost impossible.

    • Only three months remain in 2026, leaving insufficient time for an unprecedented global plague outbreak to emerge and be labeled.
    • Modern antibiotic treatments and standard infection control reliably contain pneumonic and bubonic plague outbreaks locally.
    • The WHO did not even characterize the major 2017 Madagascar outbreak as a PHEIC, let alone a worldwide pandemic.

It would require an unprecedented, fast-spreading plague event and a deliberate WHO use of “pandemic” within 85 days.

    • WHO and member-state surveillance would need to detect sustained multinational transmission, then WHO would need to use the specific label by 31 December.
    • Modern plague produces small, containable outbreaks; WHO has not characterized plague as a pandemic in its contemporary communications.
    • A confirmed multi-country pneumonic-plague outbreak with sustained human transmission would sharply raise the odds, but none is presently evident.

WHO has never called plague a pandemic, and with under three months left no outbreak is near that scale.

    • WHO's pandemic label requires sustained global spread; plague's last pandemic framing was the 14th-century Black Death, not modern IHR practice.
    • Any 2026 outbreak would first need a PHEIC declaration and weeks of escalation, and WHO has made none for plague this year.
    • Even the 2024 DRC pneumonic plague cluster stayed local; the one development that would change my mind is a multi-country sustained outbreak with WHO emergency committee…

Resolution criteria

This market will resolve to "Yes" if the World Health Organization explicitly characterizes plague, Yersinia pestis, pneumonic plague, or any plague outbreak as a "pandemic" in an official public communication between market creation and December 31, 2026, 11:59 PM ET. Otherwise, this market will resolve to "No". An explicit characterization includes official WHO statements, reports, press briefings, or publications that clearly describe the disease or outbreak as a "pandemic". A Public Health Emergency of International Concern alone will not qualify unless it is also described as a pandemic. The primary resolution source for this market will be official WHO communications; however, a consensus of credible reporting may also be used.
